Blood Transfusion Procedure

  1. Verifie doctor's order and make a treatment card.
  2. Give explanation to the client.
  3. Request blood/blood component from hospital blood bank.
  4. Prepare the equipment: blood filter/ tubing; IV solution with 0.9% NaCl; Disposable gloves; cotton balls.
  5. Warm the blood at room temperature by wrapping the blood bag in a towel.
  6. Check the venipuncture was performed with 18 or 19g angiocatheter.
  7. Inspect the IV line if flowing, obstructed or filtrated.
  8. Review baseline vital signs in client's medical record before initiating transfusion.
  9. With another registered nurse, verify the blood product compatibility to the client.
  10. Do vital signs taking before the transfusion..
  11. Give pre-med 30 minutes if any is adhered.
  12. Wash hands and don gloves.
  13. Open compatible blood aseptically and spike blood carefully, preimes tubing and remove bubbles.
  14. Disinfect the Y injection port of IV tubing and insert the needle from the BT administration set and secures adhesive tapes.
  15. Close or KVO the IV fluid while transfusion is going on.
  16. Transfuse the blood for 4-6hours at 10-15gtts, iniatially for 15 mins then regulate at ordered rate.
  17. Observe for any for any untoward signs and symptoms of allergic reaction.
  18. Swirl bag once in a while.
  19. After transfusion, close roller clamp of BT set then disconnnect from IV line then regulate the IV as regulated.
  20. Carry out post BT order such as CBC, bleeding time, etc.
  21. Document.
